Supervisory Visits

Observe the visit. Document the follow-up.

Registered nurse (RN) supervisory visits document how assigned aide services relate to the current plan of care, including patient concerns and findings requiring agency follow-up.

All services

Registered nurse (RN) supervisory visits document how assigned aide services relate to the current plan of care, including patient concerns and findings requiring agency follow-up.

Provide the current care plan, supervision requirements and any concerns your agency wants addressed. Where direct observation is needed, the coordinator must arrange a time when the aide and patient will be available.

The RN distinguishes direct observations from patient reports and documents any limitations of the visit, then verifies and signs the supervisory record. Our office reviews the documentation and coordinates any required corrections before delivery. Your agency retains responsibility for its supervision program and any corrective action or authorization required.

Visit Focus and Agency Handoff

The agreed scope guides preparation, the RN’s visit record and follow-up with your agency.

Arrange the observation

Confirm who needs to be present and which assigned tasks should be reviewed. Identify access or scheduling constraints in advance.

Record the care delivered

Compare observed tasks with the supplied care plan and hear the patient’s concerns. Distinguish direct observations from information reported by others.

Document supervisory findings

The record identifies the assigned tasks observed, patient concerns, any instructions provided and recommended agency follow-up. Tasks that could not be observed and other limits on the visit are documented for the agency’s review.

The completed visit record is delivered as a PDF or by authorized entry into your agency’s electronic health record (EHR), as agreed. Where a Plan of Care is prepared or revised, your agency’s Director of Patient Services (DPS) approves it before finalization.
