Who we are

CareBridge Staffing Network Inc., operating as CareBridge RN ('CareBridge', 'we', 'us', or 'our'), provides RN clinical assessment, documentation and coordination support to New York Licensed Home Care Agencies.

Information we collect

Our website forms collect the information you choose to provide. You do not need to create an account to use the site.

• Agency meeting bookings: contact and scheduling details you enter into Cal.com, including your responses to booking questions. Please do not include confidential patient information.
• Nurse applications (Join the Nurse Network): your name, email address, phone number, RN license status, home care experience, availability, preferred coverage area, and anything you write in the message field.

Automatic technical information

Our hosting and form-processing services may record technical information such as IP addresses, browser details, request times and errors when you use the website. This information supports website operation, troubleshooting and security.

How we use your information

We use the information you provide to:

• Respond to your inquiry and follow up about our services.
• Review and consider nurse applications.
• Communicate with you about a request you have made.
• Operate, maintain, and improve this website and our services.

How we store and share your information

Meeting bookings are processed through Cal.com, including its embedded calendar. Nurse application submissions pass through Web3Forms for delivery to CareBridge. We use Microsoft 365 / Outlook for business email. Booking and application information is used by the team handling your request; these public forms are not the channel for patient records.

We do not sell your information, and we do not share it with third parties for their own marketing. We may share it with trusted service providers who help us process bookings and form submissions and operate our email and business systems (such as Cal.com and Web3Forms), and with others when required by law or to protect our legal rights.

External services and website technologies

You can browse the public website without creating an account. Form-processing services and external websites may handle technical information under their own privacy policies.

Links that open WhatsApp or another external service take you to that provider’s service. Review its privacy settings before sharing information. We will update this policy when our website information practices change.

Health information and HIPAA

Our public website forms are intended for agency inquiries and nurse applications. Please do not submit patient names, medical records, assessment findings, or other confidential patient information through these forms.

Patient information used in agency services must be shared through the approved secure email process arranged with CareBridge, rather than public website forms. Our handling of that information is subject to applicable privacy requirements and the agency agreements and Business Associate Agreements that apply to the service.

How long we keep your information

Website inquiry and application information is retained for the purposes described in this policy and applicable legal requirements. Contact us to discuss retention or deletion of information you have submitted.

For agency services, our retention policy provides for final assessments, clinical records and clinical photographs to be kept for six years after agency discharge, or longer where applicable law, an agreement or a preservation obligation requires. Records concerning minors may require longer retention. The applicable service arrangement determines recordkeeping responsibilities.

Our retention schedule calls for temporary dictation and processing files to be deleted 30 days after final clinical approval and successful delivery, unless they are required clinical records or subject to a preservation obligation. This is separate from retention of final records. Contact us about a particular record; this schedule is not a promise of immediate removal from every backup.

SMS communications

CareBridge uses SMS text messaging as part of its business communications. Information associated with these communications may include your phone number and the messages you exchange with us.

For questions about text messages or to request that we stop contacting you by SMS, email assess@carebridgern.com. Do not send confidential patient information through ordinary SMS; use the approved secure communication process arranged with CareBridge.

Your choices and rights

You can ask us to access, correct, or delete the information you submitted, or ask us to stop contacting you, at any time. Just email assess@carebridgern.com and we will respond as required by applicable law.

Security

We use reasonable administrative, technical, and physical safeguards to protect your information. However, no method of transmitting or storing data is completely secure, so we cannot guarantee absolute security.

Children's privacy

The public website and its forms are intended for agency contacts and nursing professionals, not children. If a child’s information has been submitted here, contact us. Patient records handled separately for agency services are subject to the applicable clinical-record requirements.

Changes to this policy

We may update this Privacy Policy from time to time. When we do, we will revise the 'Last updated' date at the top of this page.
