Registered nurse assessing a patient at home
Initial / Start-of-Care Assessments

A Thorough First Visit. A Clear Plan Forward.

Establish the patient’s clinical baseline, identify care needs and build the record that guides every visit ahead. CareBridge coordinates the assessment, RN visit and documentation handoff for your agency.

All services

The first assessment shapes the entire care journey. Start with a complete clinical picture and a record your team can build on.

A start-of-care visit brings together the patient’s current condition, diagnosis information and day-to-day support needs. This comprehensive evaluation establishes a baseline and informs the individualized plan of care for subsequent visits.

Gaps in the initial record can carry through the rest of a patient’s care. A credentialed RN completes, verifies and signs the assessment. Our office supports documentation preparation, reviews the record and coordinates required corrections before delivery to your agency.

How We Coordinate Your Start-of-Care Assessment

A referral, a confirmed visit and a reviewed record give your agency the information needed for its next steps.

Prepare the referral

Share the reason for assessment, relevant history, current orders and the agency’s required forms through the document-sharing process agreed with your agency. Identify information still to be obtained.

Arrange the appointment

Our coordination team contacts the patient or family and confirms a visit plan based on access needs, availability and RN coverage.

Establish the clinical record

The RN records the patient’s condition, daily support needs and relevant findings. Medical diagnoses and orders are verified against the available record; nursing observations are documented as such.

Review and return the assessment

The RN verifies and signs the assessment. Our office reviews documentation and coordinates required corrections. Your agency remains responsible for required approvals, practitioner orders and patient notices; its Director of Patient Services (DPS) approves the Plan of Care before finalization.
