Registered nurse (RN) reassessments document changes in the patient’s condition and care needs to support care-plan review and any applicable recertification process.

Share the previous assessment, current care plan and reason the review is due through the document-sharing process agreed with your agency. Include relevant orders, reported changes and the date your agency needs the record. These documents provide a basis for comparing the patient’s present condition with the earlier assessment.

The RN verifies and signs the reassessment. Our office reviews the documentation and coordinates any required corrections before delivery. Your agency remains responsible for obtaining required practitioner orders or certification, payer recertification and service authorization.

Visit Focus and Agency Handoff

The agreed scope guides preparation, the RN’s visit record and follow-up with your agency.

Provide the earlier record

Identify the last assessment and any changes already reported. Explain which questions your agency needs the reassessment to address.

Document what has changed

The RN compares relevant symptoms, daily function, medications and support needs with the earlier record. Observed findings are distinguished from patient or caregiver reports, and information requiring further clarification is identified.

Use the reviewed findings

The reassessment provides findings for care-plan review and any applicable recertification process. It does not itself renew practitioner certification or payer authorization, or authorize a change to ordered services.

The completed visit record is delivered as a PDF or by authorized entry into your agency’s electronic health record (EHR), as agreed. Where a Plan of Care is prepared or revised, your agency’s Director of Patient Services (DPS) approves it before finalization.
