About CareBridge

We're Here to Support Your Mission.

RN clinical assessment, documentation and coordination support for New York Licensed Home Care Agencies.

Our Mission

To support New York Licensed Home Care Agencies with RN clinical assessments, reviewed documentation and coordinated visits, according to the scope agreed with each agency.

Our Story

CareBridge RN Staffing Network was founded by Nairovi Herrera, an RN with experience in home care leadership. CareBridge supports Licensed Home Care Services Agencies (LHCSAs) facing assessment demand, documentation backlogs and time-sensitive patient needs.

CareBridge was established to provide Licensed Home Care Agencies with coordinated RN assessment services, clinical documentation support and quality review. The agency retains clinical oversight, while CareBridge supports the agreed assessment and documentation responsibilities.

Why CareBridge Exists

Supporting LHCSAs through growing assessment demands and time-sensitive clinical workflows.

Licensed Home Care Services Agencies face increasing pressure to complete assessments accurately, timely, and consistently while managing growing documentation requirements and staffing challenges. CareBridge was created to help agencies expand assessment capacity while providing nurses with the support they need to succeed.

The Challenges LHCSAs Face

1

Time-Sensitive Initial Assessments

Rapid patient admissions require timely assessment completion.

2

Recertification Timeframes

Assessments must be completed within required certification periods.

3

Post-Hospitalization Assessments

Changes in condition require timely follow-up and documentation.

4

Change of Status Assessments

Agencies need responsive assessment coverage when patient needs change.

5

Documentation Burden

Documentation demands continue to increase for field nurses.

6

Nurse Capacity & Retention

Overloaded nurses can lead to delays, burnout, and turnover.
