For Home Care Agencies
Which assessment types can we request?
CareBridge supports initial, recertification, post-hospitalization, change-of-status and supervisory assessments. Discuss any additional or complex requirements with our team so we can confirm the appropriate scope.
Can we use our agency’s forms and documentation workflow?
Share your forms, documentation requirements and EHR workflow during the initial meeting. We agree how documentation will be prepared and reviewed, then deliver completed PDFs or provide authorized entry into your agency’s EHR, according to the agreed arrangement.
How quickly can an assessment be scheduled?
Tell us your requested date and whether the assessment request is time-sensitive. Scheduling depends on location, RN availability, patient availability and the information needed for the visit. Our team confirms the plan with you.
What happens if a patient cannot be reached or a visit changes?
Our coordination team works with your agency on contact attempts, rescheduling and next steps. Share changes to the patient’s availability or condition as early as possible so the visit plan can be updated.
Who reviews and approves the documentation?
Our platform prepares documentation drafts for RN review, verification and signature. Clinical review checks completeness and consistency, with questions returned to the RN for correction. We target documentation turnaround within 24 hours after the visit. Your DPS approves the plan of care before finalization.
How do we send patient information?
Use the assessment request and document-sharing process agreed with our team. The public booking form is for agency inquiries; do not include patient records or other protected health information in it.
How do pricing and ongoing support work?
We agree a per-assessment price before work begins. It includes the RN visit, scheduling coordination, documentation support, quality review and completion tracking, with no separate charges for these services and no minimum visit commitment. Discuss your visit types and requirements with our team; specific rates are shared directly.
How are the nurses credentialed?
Onboarding covers identity, New York RN licensure and current registration verified against NYSED records, relevant experience and references, required health and training records, and applicable professional liability coverage. The team confirms assignment-specific requirements and addresses missing or expired records before an assignment. See the credential checklist below for details.
What responsibilities does our agency retain?
Your LHCSA retains responsibility for service quality, clinical oversight and compliance. Your clinical leadership reviews assessments and care plans, integrates documentation into patient records and arranges required practitioner authorization. The service agreement defines CareBridge’s work and your agency’s oversight responsibilities.
What documentation can we review for audit preparation?
Assessment findings, care plans and supporting visit documents form part of the clinical record. Discuss credential evidence, record access and your agency’s audit requirements during onboarding. Our Compliance & Quality page sets out the framework and agency checklist.
Where can we view the forms included with a visit?
Open Sample Forms to explore illustrative assessment and supporting forms. We agree the required documents for each visit type with your agency. A sample package does not mean every form applies to every visit.
Locations & Scheduling
Where does CareBridge provide assessment support?
CareBridge provides RN clinical assessment support for New York Licensed Home Care Services Agencies (LHCSAs). Share the cities, counties or ZIP codes where you need visits. Our office confirms coverage based on the location, visit requirements, patient availability and approved RN availability before an assignment is scheduled.
What information do you need to review a coverage request?
Share your agency name, service ZIP codes, assessment type, expected visit volume and preferred completion dates. Include a contact person so our coordination team can discuss next steps and arrange the appropriate channel for assessment request details.
How far in advance should I request an assessment?
Contact us as soon as you know the visit is needed, and include any admission or reassessment deadline. For time-sensitive requests, highlight the requested completion date so our team can review scheduling options with you.
Can I arrange recurring coverage across multiple locations?
Yes. Share the ZIP codes, visit types and expected weekly or monthly volume for each location. Our team will review a coverage plan with you and discuss how to coordinate ongoing requests.
For Registered Nurses
How are visits assigned?
Our office coordinates patient outreach and scheduling, then matches assignments to approved nurses using availability, service areas, languages and visit requirements. This is an office-coordinated workflow rather than an open list of visits to book yourself.
Is the nurse app available now?
Yes. CareBridge’s platform supports the assessment workflow, including documentation drafts for RN review, verification and signature. Start with the public application; our office provides onboarding instructions and access. The walkthrough shows illustrative screens, not a live clinical workspace.
What happens if my documents need updates?
The office will explain what needs correction or renewal. Your profile keeps missing and expiring records visible, lets you resubmit corrections and routes changes for review. Applicable credential requirements must be satisfied before assignments.
Can I apply if I already work full-time?
Yes. Tell us whether you are looking for part-time, per-diem, weekend or other availability. Our team will discuss opportunities that fit your schedule and preferred service area.
What kinds of visits would I complete?
Our assessment work includes initial visits, recertifications, post-hospitalization assessments, changes of status and supervisory visits. Your experience and the assignment requirements guide the match.
Do I need a New York RN license?
You must be authorized to practice as an RN in the state where the assignment takes place. For our New York work, the team verifies appropriate New York licensure before assigning visits. You can share your current license status in the application.
What if I am new to home care assessments?
Include your clinical background and experience level when you apply. We will discuss the assessment responsibilities, orientation and whether available assignments are a suitable fit.
Who handles the documentation?
You complete the clinical assessment and provide the findings. Our platform prepares documentation drafts for your review, verification and signature. Our documentation team assists with preparation and corrections, followed by clinical quality review. You retain responsibility for your clinical findings and signed documentation.
How are rates and payment arrangements confirmed?
Rates are standardized by visit type and disclosed after onboarding approval, before you accept an assignment. Confirm payment timing and any applicable travel or cancellation terms with our team.
Do I need to upload documents to apply?
No documents are requested in the initial application. Start with your contact details, license status, experience and preferences. Our team will explain the next steps for providing credentials.
Who can help if a visit does not go as planned?
Your coordination team can help with scheduling changes, access issues and agency communication. During onboarding, confirm your support contacts and the process for escalating clinical concerns.
